Cognitive Functional Therapy (CFT) is a physiotherapist-led approach developed for people with persistent musculoskeletal pain, with the strongest evidence currently relating to chronic low back pain. Rather than focusing only on the painful area, it looks at how physical, psychological and lifestyle factors interact, then helps a person understand their pain, move with more confidence and return to activities that matter to them.

This guide explains what CFT is, how it works, what the research shows and how it differs from cognitive behavioural therapy (CBT). Head Agenda does not provide CFT or physiotherapy, but talking therapy may help when living with persistent pain is also affecting anxiety, mood or confidence.

What is Cognitive Functional Therapy?

CFT was developed by Professor Peter O'Sullivan and colleagues. It is an individualised approach to persistent pain in which a physiotherapist with specific CFT training builds treatment around the person's pain history, movement patterns, beliefs, emotions, daily routine and goals.

There is no fixed programme that everyone follows. One person may want to lift their children more comfortably, another may want to sleep better or return to the gym. Reducing pain can be part of the goal, but CFT places equal emphasis on function, confidence and self-management.

Why chronic pain can persist

Chronic or persistent pain is pain that lasts or recurs for more than three months. It can be secondary to an underlying condition, or it can be chronic primary pain, where no other condition fully explains the pain or its impact. The two can also exist together.

Pain is real, and its intensity is not always a direct measure of tissue damage. Sleep, stress, mood, previous experiences, expectations, activity levels and fear of movement can all influence how pain is experienced. That is why an X-ray or scan may be useful in some situations without explaining the whole picture.

When someone becomes worried that movement will cause further damage, it is understandable to become guarded or avoid activity. Over time, this can reduce strength, fitness and confidence. CFT aims to interrupt that cycle through clear explanations and carefully paced movement rather than relying on passive treatment alone.

How Cognitive Functional Therapy works

CFT targets the patterns that are most relevant to the individual. Treatment commonly brings three connected elements together.

Making sense of pain

A CFT physiotherapist explores what the person believes about their pain and body, then relates those beliefs to current pain science and their own experience. The aim is not to dismiss symptoms. It is to replace an overly threatening picture with a more accurate and useful understanding.

Reducing fear and emotional distress

Pain can affect mood, sleep and confidence, while anxiety and stress can make pain harder to manage. CFT helps a person notice these patterns and develop practical ways to respond. When the emotional impact needs dedicated support, anxiety therapy, CBT or another appropriate talking therapy may be used alongside medical or physiotherapy care.

Rebuilding confident movement

The physiotherapist observes movements that feel difficult or threatening, such as bending, lifting, sitting or walking. Together, the person and clinician practise these movements in a controlled, graduated way. The pace is adjusted to the individual so they can build confidence without treating every increase in discomfort as evidence of damage.

Why pain education matters

Education runs through the whole approach. A person may learn how the nervous system can become more protective, why pain does not always mean further injury, and how sleep, stress, activity and daily habits can affect symptoms.

This is combined with practical experience. Understanding that a movement can be safe is useful, but practising it successfully is what helps the new understanding feel credible.

Cognitive Functional Therapy vs CBT

Cognitive behavioural therapy (CBT) is a talking therapy that helps people recognise and change unhelpful patterns of thinking and behaviour across a wide range of mental and physical health concerns. It is delivered by an appropriately trained mental health professional.

CFT is delivered by a physiotherapist and is specifically organised around pain, movement and physical function. It uses psychological principles, but combines them with assessment and movement retraining in the same treatment. CFT and CBT can therefore overlap without being the same therapy.

Does Cognitive Functional Therapy work?

The evidence is strongest for chronic, disabling low back pain. The RESTORE randomised controlled trial, published in The Lancet in 2023, involved 492 adults and found that CFT, with or without movement-sensor biofeedback, improved activity limitation compared with usual care. The effects were sustained at 52 weeks.

The wider evidence still needs to be interpreted carefully. A 2023 systematic review found mixed results and judged the available studies to have a high risk of bias. A later review reached more favourable conclusions, but differences between studies remain. CFT is promising, especially for chronic low back pain, but it is not a guaranteed cure and it will not suit everyone.

Who is Cognitive Functional Therapy for?

CFT may be considered for adults with persistent musculoskeletal pain, particularly chronic low back pain that is limiting activity and has not improved through usual care. It may be especially relevant where fear of movement, unhelpful beliefs, anxiety or loss of confidence are contributing to disability.

A qualified clinician should assess persistent or changing pain and rule out serious causes before treatment begins. CFT is not the first step for sudden new pain or symptoms that may need urgent medical investigation.

What happens in a CFT session?

A first appointment is usually more detailed than a standard follow-up. The physiotherapist will ask about the pain history, general health, sleep, stress, activity, previous treatment, worries about movement and the things the person wants to return to.

Sessions may then include:

  • an assessment of how the person moves and responds to pain;
  • clear, personalised pain education;
  • guided practice of movements that feel difficult or threatening;
  • strategies for activity, sleep, stress and flare-ups; and
  • goals linked to everyday life rather than pain scores alone.

The number and spacing of sessions vary. In the RESTORE trial, participants received up to seven sessions over 12 weeks and a booster session at 26 weeks, but real-world treatment should be tailored to the individual and the service providing it.

Why the individual approach matters

Two people with similar scans can have very different symptoms, concerns and limitations. CFT uses a person-centred, biopsychosocial approach, which means it considers biological, psychological and social factors together rather than reducing the problem to one body part.

This does not mean that pain is imagined. It means the treatment plan reflects the full range of factors that can influence pain and recovery.

Finding a Cognitive Functional Therapy practitioner

CFT should be delivered by a physiotherapist who has completed specific training in the approach. Ask a prospective practitioner about their professional registration, CFT training, experience with your type of pain and how they assess whether the treatment is suitable.
